Job Summary
We are seeking a compassionate and dedicated Caregiver / Certified Nursing Assistant (CNA) to provide exceptional support and care to individuals in various settings, including in home care, assisted living, and long-term care facilities. In this vital role, you will assist our clients with daily activities, monitor their health and well-being, and ensure a safe, comfortable environment. Your energy and positive attitude will make a meaningful difference in the lives of those you serve, helping them maintain independence and dignity every day.

Duties

  • Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support and IADL'S.
  • Monitor vital signs including blood pressure, pulse, temperature, and respiration; report any abnormalities promptly
  • Support medication reminder following proper protocols and HIPAA regulations to ensure patient confidentiality
  • Provide companionship and emotional support to promote mental well-being
  • Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
  • Maintain cleanliness by assisting with light housekeeping tasks and ensuring a hygienic environment
  • Observe and document patient conditions, behaviors, and responses to care plans for accurate reporting to healthcare teams
Skills

  • Proven experience in home & community care, assisted living, nursing homes, or long-term care environments
  • Knowledge of patient observation techniques and ability to recognize changes in condition or behavior
  • Familiarity with Alzheimer’s care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities in general
  • CPR and First Aid certification to respond effectively in emergencies
  • Experience with medication administration, vital signs monitoring, and use of assistive devices such as Hoyer lifts or Sit to stand lift
  • Ability to perform heavy lifting safely when assisting with transfers or mobility support
  • Strong communication skills for collaborating with healthcare teams and family members while maintaining HIPAA compliance
  • Compassionate approach to caring for seniors and individuals requiring hospice or palliative care support
  • Ability to work independently or as part of a team in fast-paced environments like group homes or in home care settings
